Abstract Summary
Approximately 2 in 5 Canadians are expected to be diagnosed with cancer in their lifetime. As cancer incidence rises, advances in early screening and treatment have led to increased overall cancer survivor rates. Many survivors continue to be seen by specialists for routine follow-up care, which can cause delays for newly diagnosed patients and reduce access to specialized care. To reduce this burden, various shared care models between oncologists and primary care practitioners have been created and implemented across Canada. The Montreal University Health Center (MUHC) has implemented a collaborative shared care model titled ;Cancer Care Transition of Care Program (CCTCP), to facilitate the transition of follow-up care from oncologists to primary care/community providers for patients at low risk of recurrence.The study will be completed by interviewing a minimum of 6 cancer surviving patients and 6 community practitioners who participated in the program. Data will be collected during semi-structured online interviews of 45 to 60 minutes. Interviews will be transcribed verbatim and interpretive description will be used to identify prominent themes, highlight areas of satisfaction, and areas for improvement. Validated and reliable measures will be used to assess satisfaction.The aim of this qualitative/quantitative mixed-method study is to explore the experiences and levels of satisfaction of the support and guidance received for patients and community practitioners taking part in the transition to care in the community program. The analyzed data will be used to inform how the support received by patients and community practitioners can be improved to further enhance the coordination across oncology and primary care.
